Противогрибковые средства группы азолов могут увеличивать уровень сиролимуса за счет ингибирования его метаболизма, опосредованного CYP3A4 и Р-гликопротеином. Следует контролировать уровень сиролимуса в начале и при прекращении терапии азольными противогрибковыми средствами. Следует контролировать состояние пациентов и уменьшать дозу сиролимуса при необходимости.

Вориконазол может повышать уровни в сыворотке и, возможно, токсичность сиролимуса, подавляя его СУР3А4-опосредованный метаболизм и/или Р-гликопротеиновый транспорт. Сообщали о повышении AUC сиролимуса в 11 раз и токсичности (панцитопения, нарушение функции почек, гиперлипидемия) у пациента при комбинированном применении этих препаратов. Однако в нескольких исследованиях на аллогенных гемопоэтических стволовых клетках реципиента трансплантата обнаружили, что уменьшение эмпирической дозы сиролимуса на 90% приводило к аналогичному уровню сиролимуса при предварительном применении вориконазола. Одновременное применение вообще не рекомендуется. Если комбинации не избежать, следует уменьшить дозу сиролимуса, как рекомендовано, и тщательно контролировать его уровень в сыворотке крови и откорректировать дозу соответственно.
