Описание взаимодействия
Рифамицины являются индукторами CYP3A4, ингибиторы протеазы - ингибиторами. При совместном применении концентрации ингибиторов протеаз могут снижаться до субтерапевтических значений, а концентрация рифамицинов – повышаться до токсических. Применение рифампицина с ингибиторами протеазы не рекомендуется, из-за вероятности снижения противовирусного действия. Рифабутин (более слабый индуктор CYP3A4) может применяться с большей частью ингибиторов протеазы, но дозы рифабутина должны быть снижены. Применение рифабутина с саквинавиром не рекомендуется из-за значительного снижения концентрации саквинавира, за исключением случаев, когда саквинавир назначается с ритонавиром.
Рекомендации
Применение такой комбинации требует особой осторожности.
Комбинации лучше избегать, за исключением случаев, когда предполагаемая польза превышает вероятный риск.

Рифампицин может снижать концентрации в сыворотке крови и, возможно, противовирусную эффективность ингибиторов протеазы, индуцируя их СУР3А4-опосредованный метаболизм. Несколько исследований обнаружили, что одновременное применение рифампицина значительно снижало AUC и Cmin ингибиторов протеаз. Увеличение дозы ритонавира или обоих средств с целью компенсировать фармакокинетическое взаимодействие привело к росту частоты развития гепатотоксичности и побочных эффектов со стороны желудочно-кишечного тракта у здоровых добровольцев, но лучше переносилось ТБ/ВИЧ ко-инфицированными пациентами. С другой стороны, индинавир и атазанавир повышали уровень рифампицина. Одновременное применение не рекомендуется. Если комбинированное использование является неизбежным, необходимо внимательно следить за функциями печени пациента, особенно при увеличении дозы ингибитора протеазы.
