Противогрибковые средства группы азолов могут увеличивать уровень сиролимуса за счет ингибирования его метаболизма, опосредованного CYP3A4 и Р-гликопротеином. Следует контролировать уровень сиролимуса в начале и при прекращении терапии азольными противогрибковыми средствами. Следует контролировать состояние пациентов и уменьшать дозу сиролимуса при необходимости.

Итраконазол может увеличивать уровни в сыворотке и, возможно, усиливать токсичность сиролимуса, подавляя его СУР3А4-опосредованный метаболизм и/или Р-гликопротеиновый транспорт. Сообщали о девятикратном повышении уровня в сыворотке сиролимуса при комбинированном применении. Кроме того, у пациента с пересадкой сердца требовалось удвоение дозы сиролимуса после прекращения приема итраконазола для поддержания терапевтического уровня сиролимуса. Следует тщательно контролировать уровни сиролимуса в сыворотке и соответствующим образом корректировать его дозу. Рекомендуется уменьшить дозу сиролимуса на 50-90% в начале комбинированной терапии с итраконазолом.
