Совместимость Рифабутин и Норвир^®

Рифамицины являются индукторами CYP3A4, ингибиторы протеазы - ингибиторами. При совместном применении концентрации ингибиторов протеаз могут снижаться до субтерапевтических значений, а концентрация рифамицинов – повышаться до токсических. Применение рифампицина с ингибиторами протеазы не рекомендуется, из-за вероятности снижения противовирусного действия. Рифабутин (более слабый индуктор CYP3A4) может применяться с большей частью ингибиторов протеазы, но дозы рифабутина должны быть снижены. Применение рифабутина с саквинавиром не рекомендуется из-за значительного снижения концентрации саквинавира, за исключением случаев, когда саквинавир назначается с ритонавиром.

Ингибиторы протеазы, особенно усиленные ритонавиром, могут ингибировать СУР3А4-опосредованный метаболизм рифабутина и его активного метаболита (25-О-дезацетилрифабутина), в результате чего повышается уровень в сыворотке крови и токсичность рифабутина. Сообщали о повышении уровня рифабутина и 25-О-дезацетилрифабутина с последующим увеличением их токсических эффектов, если доза рифабутина снижалась. Кроме того, рифабутин является слабым индуктором изофермента СУР3А4 и может повышать метаболизм ингибиторов протеазы. Однако влияние на содержание в сыворотке крови ингибиторов протеазы (кроме саквинавира), как правило, было незначительным. Уменьшить дозу рифабутина и следить у пациента за признаками токсичности рифабутина при комбинированном применении с ингибиторами протеазы.
