Совместимость Рифапекс и Норвир

Рифамицины являются индукторами CYP3A4, ингибиторы протеазы - ингибиторами. При совместном применении концентрации ингибиторов протеаз могут снижаться до субтерапевтических значений, а концентрация рифамицинов – повышаться до токсических. Применение рифампицина с ингибиторами протеазы не рекомендуется, из-за вероятности снижения противовирусного действия. Рифабутин (более слабый индуктор CYP3A4) может применяться с большей частью ингибиторов протеазы, но дозы рифабутина должны быть снижены. Применение рифабутина с саквинавиром не рекомендуется из-за значительного снижения концентрации саквинавира, за исключением случаев, когда саквинавир назначается с ритонавиром.

Рифапентин может снижать концентрацию в сыворотке крови и, возможно, противовирусную эффективность ингибиторов ВИЧ-протеазы, индуцируя их СУР3А4-опосредованный метаболизм. В исследовании фармакокинетики установили, что рифапентин снижал AUC и Cmax индинавира на 70% и 55% соответственно. Кроме того, рифапентин не рекомендуется пациентам с латентным или активным туберкулезом на антиретровирусной терапии. Комбинированное применение противопоказано.
